What Happened

Major depression affects 5% of the global population and is the fourth leading cause of disability. Symptoms include profound grief, sadness, anhedonia, delusional anti self confabulation, and vegetative symptoms, such as chronic exhaustion and sleep disturbances.

Anhedonia, or the inability to experience pleasure, often manifests in a flat emotional affect. Delusional anti self confabulation involves the creation of unfounded negative self-perceptions. Vegetative symptoms are characterized by constant fatigue and disrupted sleep patterns, with physiological markers like a peak in cortisol levels around 9pm.

Traditional antidepressants, such as tricyclics, MAO inhibitors, and SSRIs, target neurotransmitters like norepinephrine, dopamine, and serotonin, each linked to different aspects of depression. However, about one-third of individuals taking SSRIs do not find relief, pointing to the need for alternative treatments.

Hormonal changes, such as those occurring during the menstrual cycle, postpartum periods, and menopause, can influence depressive states, highlighting the complex interaction between hormones and mood. Stress also plays a significant role in depression, with a correlation between repeated intense stress episodes and increased depression risk.

The genetic component is evident, as identical twins show a 50% probability of both experiencing major depression if one is affected. Chronic inflammation, mediated by cytokines like IL6 and TNF alpha, is associated with depression, suggesting a link between immune response and mood disorders.

Emerging treatments, like ketamine and psilocybin, are being studied for their potential to treat depression by altering neural circuits and neurotransmitter activity. Lifestyle interventions, such as exercise and dietary changes like the ketogenic diet, can also support mental health by influencing neurotransmitter levels and inflammatory response.
